On the upper left corner place your name and section. For Example:

Then centered in the middle of the page is,

On the lower right hand corner place the date of submission and the teacher's name. Like so:

Place which quarter the activity belongs to. For Example:

1st Quarter

Please repeat this for all quarters and place it before its corresponding activities.

Activity/Experiment Page

On the upper left corner place the topic and page number the activity/experiment is found in your Geometry books. For Example:

Sum of the Exterior Angles of a Polygon
p. 517

On the bottom of the page make your conjecture on the activity/experiment you did. Like so:

Exterior Angle Sum Theorem: If a polygon is convex, then the sum of the measures of the exterior angles, one at each vertex, is 360.

Last Page

Make a two paragraph essay with seven sentences each, answering the question, "How did Modeling Math activities help you in understanding the lesson?".
